This incident report MUST be filled out by the student and a copy is turned in to the library, security, and administration. It is CRITICAL that this form be filled out immediately when the student realizes that Chromebook/Device is lost or has been stolen. This form needs to be filled out ANY time a device has been lost/stolen. It needs to be on file in the library. Form needs to be signed and printed by parent/guardian and student. Print and turn in. If you would like a copy of the completed form, we can make copies in the library. Thank you.
This form needs to be filled out by student for any repair done to Chromebook/laptop. It must also be signed by the student and the parent. Form can be printed and filled out, it must be filled out, signed, and returned to the library along with the Chromebook/laptop.
